CLIENT SERVICE AGREEMENT

Should you choose to engage with any of my services, these are the terms and conditions:
Services Provided
As a certified health coach, I, Susanne Cesana, provide wellness coaching, education, and support to help clients make informed decisions about their health and lifestyle.
Scope of Practice
While my coaching focuses on supporting metabolic health and overall wellness, I am not a doctor, registered dietitian, or licensed healthcare provider. My services do not include:
- Diagnosing, treating, or curing prediabetes, diabetes, or any other medical condition
- Prescribing medications, supplements, or medical interventions
- Replacing medical advice from a doctor, endocrinologist, or other licensed provider
Clients are responsible for their own health decisions. Before making significant changes to diet, exercise, or other lifestyle factors, clients should consult with their primary care provider or other qualified healthcare professional, especially if they:
- Have been diagnosed with diabetes, prediabetes, or any other medical condition
- Are taking medication that affects blood sugar (such as insulin or metformin)
- Have dietary restrictions or special medical needs
- Are pregnant, breastfeeding, or under medical supervision
By engaging in coaching services, the client acknowledges that:
- Coaching is not a substitute for medical care.
- Any lifestyle changes are made at their own discretion and risk.
- Susanne Cesana is not liable for any personal outcomes, health conditions, or complications that arise from implementing coaching recommendations.
Metabolic Momentum Community — Monthly Subscription
If you are enrolling in the Metabolic Momentum Community monthly membership, the following terms apply in addition to the general terms of this agreement.
What Is Included
Your monthly membership includes weekly live group coaching calls, access to the resource library of recorded calls and videos, and additional resources as developed and announced.
Recurring Billing Authorization
By checking the subscription consent box at checkout, you authorize Empowered Lifestyle, LLC to charge your payment method at the current monthly rate on your enrollment date each month. This charge recurs automatically until you cancel. The current monthly rate is displayed at the time of enrollment and confirmed in your welcome email.
Checkout Consent — What You Agreed To
At checkout, you confirmed the following by checking the subscription consent box:
- The Metabolic Momentum Community is a recurring monthly membership billed at $49/month.
- Your payment method will be charged automatically each month on your enrollment date until you cancel.
- You may cancel at any time through the Stripe Customer Portal (link in your welcome email) or by emailing susanne@empoweredlifestylect.com.
- Monthly payments are non-refundable, but you retain full access through the end of your paid month.
How to Cancel
Cancellation takes effect at the end of your current billing period. You will not be charged for the following month.
- Online (preferred): Use the Stripe Customer Portal link in your welcome or confirmation email to cancel in one step — no phone call required.
- By email: Email susanne@empoweredlifestylect.com with your name and account email address, and your cancellation will be processed promptly.
Price Changes
You will receive at least 30 days’ advance notice by email before any price change takes effect and may cancel before the new rate applies.
Commitment & Payment — Coaching Programs and Packages
By enrolling in any coaching program or package (other than the monthly subscription described above), you acknowledge that:
- All payments are final, non-refundable, and non-transferable.
- There are no cancellations or chargebacks allowed once payment is made.
- Full payment is required before services begin.
No Refunds, No Cancellations — Coaching Programs
By purchasing a coaching program, you fully understand and agree that:
- Once a coaching program has been purchased, you are fully responsible for the payment and participation.
- There will be no refunds, cancellations, or early termination of services, regardless of personal circumstances, scheduling conflicts, or perceived progress.
- Failure to attend scheduled sessions does not entitle you to a refund or reschedule beyond the terms initially agreed upon.
Financing Options
Empowered Lifestyle, LLC may offer third-party financing options (such as Klarna or similar services) for certain programs. Financing is provided by and governed solely by the terms of the applicable third-party lender. Empowered Lifestyle, LLC is not a party to your financing arrangement and has no liability for its terms or decisions. Regardless of your chosen payment method, Empowered Lifestyle, LLC’s no-refund policy applies in full. Approval for financing is not guaranteed and is determined solely by the third-party provider.
Rescheduling Policy
If an appointment needs to be rescheduled, the request must be made at least 48 hours prior to the scheduled appointment time. Failure to provide at least 48 hours’ notice may result in the session being forfeited without the opportunity for rescheduling.
Chargeback Policy
By agreeing to these terms, you expressly waive any right to dispute or request a chargeback for services rendered.
- Any chargeback initiated for a valid purchase will be considered a breach of this agreement, and legal action may be taken to recover the full payment plus any associated fees.
- If a chargeback is initiated, your access to all services, materials, and programs will be immediately revoked.
Client Responsibilities
To ensure the best possible results, you agree to:
- Be proactive, engaged, and committed to your coaching program.
- Follow through on recommendations, assignments, and action plans.
- Communicate openly about any concerns or questions.
Results Disclaimer
You acknowledge that:
- I do not guarantee specific outcomes, as results vary based on individual circumstances, effort, and adherence to coaching strategies.
- You are solely responsible for your choices, actions, and progress throughout the program.
Confidentiality
Your personal information and coaching discussions will remain strictly confidential, except in cases where disclosure is required by law. For more details on how your information is handled, please refer to my Privacy Policy.
Intellectual Property
All materials, resources, and content provided during coaching sessions are my intellectual property and are for your personal use only. You may not reproduce, distribute, or share any content without my express written permission.
Text Messaging & Communication
SMS opt-in is voluntary and is not required to purchase any service or membership. If you separately opt in to receive text messages, you consent to receive communications related to coaching services, appointment reminders, and program updates. Standard message and data rates may apply. You may opt out at any time by replying STOP or by contacting susanne@empoweredlifestylect.com. Your phone number will not be shared or sold.
Governing Law & Dispute Resolution
This agreement is governed by the laws of Connecticut, United States. Any disputes arising from this agreement shall be resolved through mediation first, and if necessary, legal proceedings in a court of competent jurisdiction.
Acknowledgment & Agreement
By purchasing a service or program, you confirm that you have read, understood, and agreed to these terms in full. For subscription enrollments, your agreement is additionally confirmed by checking the subscription consent box at checkout. You acknowledge that you are voluntarily entering into this agreement without coercion.
